- Go to the Education tab on the left panel. Next, open Spiritual Assessment and select Questions, where you can manage Questions.
- On the top right corner you will find an option named Add Question click on it.
1. Questionnaire
Select the questionnaire to which this question will belong. The list displays available questionnaires in the system.
2. Question Set
Select the question set to which this question will belong. The list displays available question sets in the system.
3. Question
Add questions.
- Click the “X” icon to remove a question.
- Click “+ Add More” to add additional custom question fields.
