- Step 1: General Info
This section captures the basic details of the question set.
1. Questionnaire
Select the questionnaire to which this question set will belong. The list displays available questionnaires in the system.
2. Question Set
Enter the name/title of the new question set. This helps identify the set within the selected questionnaire.
3.Customize Question (Optional)
Add custom questions specific to this question set.
- Click the “X” icon to remove a question.
- Click “+ Add More” to add additional custom question fields.
Step 2: Grading
This section defines the scoring and grading structure for the question set.
1. Total Grading
Enter the total possible score for the question set.(if you set total grading 3, user can see the following)
2. Customise Text
Define grading labels or feedback text for specific score ranges (e.g., Excellent, Good, Needs Improvement).
- Use the “X” icon to remove an entry.
- Click “+ Add More” to add additional grading ranges.
3. For Total Of (Range Fields)
Specify the score range associated with the corresponding custom text.
Enter the minimum score in the first field.
Enter the maximum score in the second field.
Example:
- 80 to 100 → “Excellent”
- 60 to 79 → “Good”
